Voter Registration The Higher Education Act Amendments of 1998 requires universities to make a “good faith effort” to make voter registration forms available to you. Voter registration forms are available to Ave Maria University students in the Office of Academic Records during normal business hours. Please see the website of the U. S. Election Assistance Commission for further information regarding voter registration, at http://www.eac.gov. Interested parties can select to register to vote by mail at https://www.eac.gov/voters/national-mail-voter-registration-form/ Veterans Affairs The Office of Academic Records provides services to U. S. veterans, servicemembers, and their dependents in its capacity as Certifying Official. As a University that has been approved for Veteran training by the State Approving Agency in Florida, Ave Maria University is honored to assist those who have served and are serving our country. The Office of Academic Records is committed to aiding prospective and current veteran students in applying for and maintaining their VA Education Benefits. The Post 9/11 Veterans Education Assistance Act of 2008 Also known as “Chapter 33,” the Post 9/11 Veterans Education Assistance Act of 2008 is, as the VA says, “the most comprehensive education benefit package since the original GI Bill® was signed into law in 1944.” Parties interested in applying for and receiving Chapter 33 benefits must have served on active duty for at least 90 aggregate days after September 10th, 2001. There are also provisions made for individuals honorably discharged for a service-connected disability following 30 continuous days of active service following September 10th, 2001. Eligibility information, charts comparing Chapter 33 benefits to the existing benefit programs, and a link to the VONAPP (Veterans Online Application) are all available on the GI Bill® website, www.gibill.va.gov. The VA is currently accepting applications for Chapter 33 benefits. If you are currently receiving benefits under other Chapters (30, 35, 1606, or 1607), please contact the Office of Academic Records at (239) 280-2565 or registrar@avemaria.edu to discuss the positives and negatives to making an irrevocable switch to Chapter 33. Chapter 33 is also unique in that, for the first time in history, active-duty servicemembers can now transfer their benefit eligibility to their dependents, either spouse or children. The application to transfer benefits is handled through the Department of Defense, not the Department of Veterans Affairs. “Yellow Ribbon” The Yellow Ribbon program is a source of supplemental funding for eligible Veteran students receiving Chapter 33 benefits. The purpose of Yellow Ribbon is to allow Veteran students to attend any institution of higher education, public or private, at little to no cost to them. Beginning with the 2009-2010 academic year (the first year Chapter 33 and Yellow Ribbon benefits were made available to eligible Veteran students), Ave Maria University has been privileged to partner with the VA in offering this additional assistance to our Veterans. Other Chapters of Benefits Ave Maria University has been approved for Veteran training by the State Approving Agency (State of Florida). Veterans eligible for other benefits chapters (30, 32, 35, 1606, or 1607) are also welcome to study at Ave Maria University, where they may apply their education benefits. Resources & Information Department of Veterans Affairs Homepage for Education Benefits Web Automated Verification of Enrollment (W.A.V.E.) for Veterans FERPA Information Notification of Rights under the Family Educational Rights and Privacy Act (FERPA) The Family Educational Rights and Privacy Act of 1974 (FERPA) affords students certain rights with respect to their education records. In accordance with FERPA regulations, Ave Maria University (AMU) will not release a student’s confidential personal information to a third party without the written consent of the student. In addition, Ave Maria University has established the following policy regarding student academic records: The right to inspect and review the student’s education record within 45 days of the day the University receives a request for access.Students should submit to the registrar written requests that identify the record(s) they wish to inspect. The registrar will make arrangements for access and notify the student of the time and place where the records may be inspected. The right to request the amendment of the student’s education records that the student believes are inaccurate, misleading, or otherwise in violation of the student’s privacy rights under FERPA.Students may ask the University to amend a record that they believe is inaccurate or misleading. They should write the University official responsible for the record, clearly identify the part of the record they wanted changed, and specify why it is inaccurate or misleading.Should the University decide not to amend the record as requested, the University will notify the student in writing of the decision and the student’s right to a hearing regarding the request for amendment. Additional information regarding the hearing procedures will be provided to the student when notified of the right to a hearing. The right to provide written consent before the University discloses personally identifiable information from the student’s education records, except to the extent that FERPA authorizes disclosure without consent. The University discloses education records without a student’s prior written consent under the FERPA exception for disclosure to school officials with legitimate educational interests. A school official is a person employed by the University in an administrative, supervisory, academic or research, or support staff position (including law enforcement unit personnel and health staff); a person or company with whom the University has contracted as its agent to provide a service instead of using University employees or officials (such as an attorney, auditor, or collection agent); a person serving on the Board of Trustees; or a student serving on an official committee, such as a disciplinary or grievance committee, or assisting another school official in performing his or her tasks. A school official has a legitimate educational interest if the official needs to review an education record in order to fulfill his or her professional responsibilities for the University. Effective October 26, 2001, institutions are permitted to disclose-without the consent or knowledge of the student or parent-personally identifiable information from the student’s education records to representatives of the Attorney General of the United States in response to an ex parte order in connection with the investigation or prosecution of terrorism crimes (USA PATRIOT ACT of 2001). An ex parte order is an order issued by a court without notice to an adverse party. In addition, when the University makes a disclosure pursuant to an ex parte order, the University official is not required to record that disclosure of information in the student’s file. As of January 3, 2012, the U.S. Department of Education’s FERPA regulations expand the circumstances under which your education records and personally identifiable information (PII) contained in such records — including your Social Security Number, grades, or other private information — may be accessed without your consent. First, the U.S. Comptroller General, the U.S. Attorney General, the U.S. Secretary of Education, or state and local education authorities (“Federal and State Authorities”) may allow access to your records and PII without your consent to any third party designated by a Federal or State Authority to evaluate a federal- or state-supported education program. The evaluation may relate to any program that is “principally engaged in the provision of education,” such as early childhood education and job training, as well as any program that is administered by an education agency or institution. Second, Federal and State Authorities may allow access to your education records and PII without your consent to researchers performing certain types of studies, in certain cases even when we object to or do not request such research. Federal and State Authorities must obtain certain use-restriction and data security promises from the entities that they authorize to receive your PII, but the Authorities need not maintain direct control over such entities. In addition, in connection with Statewide Longitudinal Data Systems, State Authorities may collect, compile, permanently retain, and share without your consent PII from your education records, and they may track your participation in education and other programs by linking such PII to other personal information about you that they obtain from other Federal or State data sources, including workforce development, unemployment insurance, child welfare, juvenile justice, military service, and migrant student records. The right to file a complaint with the U.S. Department of Education concerning alleged failures by Ave Maria University to comply with the requirements of FERPA. Ave Maria University designates the following as Directory Information: The student’s name, addresses, telephone number, e-mail address, date and place of birth, major field of study, degree sought, grade level, expected date of completion of degree requirements and graduation, degrees and awards received, dates of attendance, full or part time enrollment status, the previous educational agency or institution attended, class rosters, participation in officially recognized activities and sports. Students may restrict the release of Directory Information, except to school officials with legitimate educational interests. To do so, a student must make the request in writing to the Office of Academic Records. The request must be submitted annually within one week of the first day of classes each academic year. RELEASE OF CONFIDENTIAL INFORMATION Form Transfer of Credit Policies Credit earned at other, accredited, post-secondary institutions can be applied to Ave Maria University requirements, provided the student earned at least a “C-” in each course proposed for transfer. When students from other institutions apply to Ave Maria University their official transcripts should be sent to the Office of Admissions. Upon acceptance, unless otherwise desired in advance, Admissions will send their transcripts and a course catalogue and/or course syllabi to the Office of Academic Records for evaluation. Transfer coursework is evaluated for correspondence with Ave Maria University courses and applied towards the student’s program of studies. “Total accepted transfer hours are shown on the transcript. Neither transfer credit hours nor grades are used in calculating cumulative grade point averages. Transfer students must be in residence for their final year and complete at least 32 credit hours of coursework at Ave Maria University in order to earn a degree from this institution. ” (taken from the Ave Maria University Academic Catalogue, Academic Policies section.) In order to earn major specific or core class credit, students must earn a 4 for HL IB or a 5 for SL IB. For AICE, a score of a d is required. For general elective credit, a score of a 3 for HL IB or a 4 for SL IB will be accepted; a score of an e for AICE is required. Before reviewing any IB or AICE classes, Ave Maria will need syllabi for the corresponding courses and the official score report. While the Section Search function is accessible to the general public at all times, only students who have been approved by their academic advisors for registration will be able to select courses and register for classes. (Please review the academic calendar for the specific dates) Directed Independent Study Form: This form must be completed, signed by all required parties, and submitted to the Office of the Registrar no later than the last day to Add courses. Internship Request Form: This form is used to apply for permission to add an internship course, not a directed study course. This form must be submitted before the last day to add course. Transient Student Form/External Coursework Approval Form: For use by matriculated, current students only seeking permission to take courses at other institutions and apply that credit to their Ave Maria University program of study. Request to Audit a Course: This form is used to request to audit a class. Forms for Changes to Student Information/Status Major, Minor Declaration Form: Students are required to complete and submit this form during their Sophomore year. This form may also be used to indicate a new major, double major, minor, multiple minors. Advisor Change Form: This form may be used to request a change to your advisor. Institutional Withdrawal & Exit Interview: This form is completed if you do not plan to return to AMU and/or intend to leave the university during the course of the semester. You may also fill out this form if you intend to leave the University during the ADD/DROP period of the semester. Approval to Take 500 Level Course as an Undergrad: This form is required for any undergraduate student who wants to take any THEO class coded 500 level and above. Application to Graduate Forms Graduation is not automatic! You must complete the appropriate Application to Graduate form in order to be included in graduation ceremonies and for successful processing of your degree and reception of your diploma. Failure to do so within the time communicated to you by the university will delay processing. Upon receipt of the Application to Graduate, the Office of Academic Records will complete a Degree Audit. If any discrepancies are noted, you will be contacted via your Ave Maria email. Degree Audit Request. This form provides the student a detailed academic plan and is used for requesting a degree audit.
