Application To Graduate Forms
Graduation is not automatic!
You must complete the appropriate Application to Graduate form in order to be included in graduation ceremonies and for successful processing of your degree and reception of your diploma. Failure to do so within the time communicated to you by the university will delay processing.
Upon receipt of the Application to Graduate, the Office of Academic Records will complete a Degree Audit. If any discrepancies are noted, you will be contacted via your Ave Maria email.
Degree Audit Request. This form provides the student a detailed academic plan and is used for requesting a degree audit.
Voter Registration
The Higher Education Act Amendments of 1998 requires universities to make a "good faith effort" to make voter registration forms available to you. Voter registration forms are available to Ave Maria University students in the Office of Academic Records during normal business hours. Please see the website of the U.S. Election Assistance Commission for further information regarding voter registration, at http://www.eac.gov. Interested parties can select to register to vote by mail at https://www.eac.gov/voters/national-mail-voter-registration-form/

FORMS FOR REGISTRATION
Course Withdrawal Form: For use when you wish to withdraw from a course after the last day to Drop but before or on the last day to withdraw without penalty. (Please review the academic calendar for the specific dates)
Directed Independent Study Form: This form must be completed, signed by all required parties, and submitted to the Office of the Registrar no later than the last day to Add courses.
Internship Request Form: This form is used to apply for permission to add an internship course, not a directed study course. This form must be submitted before the last day to add course.
Transient Student Form/External Coursework Approval Form: For use by matriculated, current students only seeking permission to take courses at other institutions and apply that credit to their Ave Maria University program of study.
Approval to Take 500 Level Course as an Undergrad: This form is required for any undergraduate student who wants to take any THEO class coded 500 level and above.
Approval to take 500 level CRW course as an Undergrad: This form is required for any undergraduate student who wishes to register for any CRW class coded as a 500 level course.
Approval to take 500 level PHIL course as an Undergrad: This form is required for any undergraduate student who wishes to register for any PHIL class coded at 500 level.
Request to Audit a Course: This form is used to request to audit a class. No grad, nor GPA credits will be added to the student's transcripts.
FORMS FOR CHANGES TO STUDENT INFORMATION/STATUS
Major, Minor Declaration Form: Students are required to complete and submit this form during their Sophomore year. This form may also be used to indicate a new major, double major, minor, multiple minors.
Advisor Change Form: This form may be used to request a change to your advisor.
Institutional Withdrawal & Exit Interview: This form is completed if you do not plan to return to AMU and/or intend to leave the university during the course of the semester. You may also fill out this form if you intend to leave the University during the ADD/DROP period of the semester.

Self Service Registration
During open registration periods, students can search for and register for their classes using Ave Maria University's SelfService Portal. While the Section Search function is accessible to the general public at all times, only students who have been approved by their academic advisors for registration will be able to select courses and register for classes.
Please keep in mind that the Office of Academic Records does not have access to reset SelfService usernames or passwords; please contact Alex Chaparro Systems and Engineering office (extension 2585; (239) 280-2585) for assistance.

For a full listing of courses currently on offer, select the appropriate "Period" from the drop-down menu and "Session" 01, then "Search." Summer term has two sessions, 01 and 02. To sort both, leave the session blank.
