Ave Maria University Immunization Requirements

Ave Maria University requires that ALL students provide evidence of MMR (Measles, Mumps, Rubella), Hepatitis B, and Meningococcal immunization, or else submit a signed waiver declining receipt of vaccinations, after being made aware of the risks associated with these diseases. Completed vaccination usually involves a series of two shots. Completed vaccination for Hepatitis B usually involves a series of three shots.

Additionally, international students must provide evidence of tuberculosis screening prior to arrival at AMU.

Required Health Forms for NEW Students

All incoming students must submit the following health forms online before they arrive on campus. Failure to complete these forms by the appropriate deadline will result in a hold being placed on your student account, preventing registration for courses.

You must login using your Ave Maria sign-on; if you have any trouble logging in, please contact the IT Helpdesk at 239-280-2585 or IThelpdesk@support.avemaria.edu.

The forms that must be completed are as follows; please be sure to read each form carefully to ensure that it is properly completed. Forms that are not properly completed will be rejected.

  1. Confidential Health Record Form
  2. Emergency Contact Form
  3. Mandatory Immunization Form
  4. Health Insurance Form

— Note: If you are an F-1 visa student, you will receive information regarding your enrollment in the University’s International Student Health Insurance plan. If you have any questions or concerns regarding International Student Health Insurance, please contact the Office of Student Affairs at StudentAffairs@avemaria.edu.

If you are under the age of 18 at time of completion, you will be prompted to enter an email address for your parent/guardian so that they may sign your forms, as well.

Additional Forms for NEW Varsity & Non-Varsity Student Athletes

In addition to the forms required by all students, all new student athletes must also complete the following forms prior to arriving on campus. Students who neglect to complete their health forms in a timely fashion will not be allowed to participate in practices until those forms are submitted.

  1. Acknowledgment of Insurance Requirements
  2. Athlete Physical Exam by MD, DO, or PA
  3. Assumption of Risk, Release of Medical Information, and Authorization to Obtain Medical Information Form
  4. Drug Testing Consent
  5. PPE Questionnaire
  6. Sickle Cell Trait Status Form

If you are under the age of 18 at time of completion, you will be prompted to enter an email address for your parent/guardian so that they may sign your forms, as well.

FAQs

What should I do if I have not completed the MMR, Hepatitis B, or Meningococcal vaccines?

Please review and complete the AMU Immunization Waiver Form and include it as an attachment with your Immunization Form submission. Please note: If you are under 18 years of age at the time of submission, the waiver must be signed by your parent/guardian.

What is an acceptable file type for uploading documents?

We accept the following file types: PDF, PNG, JPG, JPEG, and GIF. Normally, photos taken on a phone in one of these formats are fine, as long as all information indicated above can be seen clearly in your photos.

What counts as an “acceptable immunization record” that can be submitted in place of the completed AMU Immunization History Form?

An acceptable immunization record should have two identifiers (i.e. patient name and date of birth), plus type, date, and outcome of each vaccine/titer administered. There is no set format for these records, but the source must be one of the following:

    • A copy of your original immunization records.
    • An immunization record issued by a clinic, physician’s office, or state health department.
    • An immunization record from another school, provided it is on official letterhead from the school.

Does the AMU Immunization History Form have to be completed by a healthcare provider?

Yes.

What happens if I fail to submit my health forms prior to arriving on campus or enrolling in courses?

A hold will be placed on your student account preventing registration for the following term until you have completed submission of your health forms.
