Enrollment and Degree Verification

Transcripts cannot be issued without the student’s written permission (FERPA.1974). All financial obligations to the University must be resolved before a transcript will be released. Students on the Stop List for account balance are unable to receive official transcripts until balance is resolved with the Business Office. Ave Maria University policy does not permit this office to send transcripts (either official or unofficial) by fax or to disclose grades over the telephone. A student’s transcript is a full account of his/her academic progress at Ave Maria University, including all courses taken, grades earned, degrees conferred and honors achieved (if applicable). As the successor institution to Ave Maria College (AMC), Ave Maria University assumes the responsibility of issuing AMC transcripts. AMC transcripts appear on Ave Maria University transcript paper. Students who attended Ave Maria College of the Americas (AMCA) or Ave Maria University-Latin American Campus (AMULAC) may request their transcripts from the AMU Florida campus. Transcripts appear on Ave Maria University transcript paper.
An electronic delivery option exists for transcripts via eScripSafe by providing the email address of the transcript recipient (not available for College of the Americas students). Please note: Ave Maria University does not email your actual transcript; rather, an encrypted PDF file is submitted from Ave Maria to a secure server; the recipient is then notified by email with a one-time use link to present credentials, log in to the system, and access the transcript. The PDF file will automatically delete 24 hours after it is first opened.

Students from the College of the Americas are unable to request electronic transcript delivery, but are able to order and make payment online.
- Transcript Request by mail or in person: download form Payment will be accepted in the form of cash, check, or money order. Please mail to: Office of Academic Records, Ave Maria University, 5050 Ave Maria Blvd., Ave Maria FL 34142. Transcript requests cannot be processed until payment is received.

For a full listing of courses currently on offer, select the appropriate “Period” from the drop-down menu and “Session” 01, then “Search.” Summer term has two sessions, 01 and 02. To sort both, leave the session blank.

During open registration periods, students can search for and register for their classes using Ave Maria University’s SelfService Portal. While the Section Search function is accessible to the general public at all times, only students who have been appoved by their academic advisors for registration will be able to select courses and register for classes.
Please keep in mind that the Office of Academic Records does not have access to reset SelfService usernames or passwords; please contact Alex Chaparro Systems and Engineering office (extension 2585; 239-280-2585) for assistance.

Forms for Registration
Add/Drop Form: For use when you wish to add or drop a course if you are unable to do so via Self Service.
Course Withdrawal Form: For use when you wish to withdraw from a course after the last day to Drop but before or on the last day to withdraw without penalty. (Please review the academic calendar for the specific dates)
Directed Independent Study Form: This form must be completed, signed by all required parties, and submitted to the Office of Academic Records no later than the last day to Add courses.
Transient Student Form/External Coursework Approval Form: For use by matriculated students only seeking permission to take courses at other instutitons and apply that credit to their Ave Maria University program of study.
Forms for Changes to Student Information/Status
Declaration of Major/Minor: Students are asked to complete and submit this form during their Sophomore year. This form may also be used to indicate a double major, minor, or multiple minors. If you have already declared one major and wish to add a double major, please simply add as “second major” on this form.
Institutional Withdrawal Form: This form is only to be used by students who are withdrawing from Ave Maria University, not simply withdrawing from a class. Please note: if you intend to finish the current term and then depart Ave Maria University, please complete and submit this form at the end of the semester.
Change of Address Form: This form must be completed and submitted within 10 days of a student’s move to ensure proper record maintenance.
Application to Graduate Forms
Graduation is not automatic! You must complete the appropriate Application to Graduate form (found below or in the Office of Academic Records) and submit it no later than the semester immediately preceding the semester in which you will graduate. The form can be turned in to the Office of Academic Records (room 103, Student Enrollment Center), or emailed to firstname.lastname@example.org. Faxed applications will also be accepted; the number is 239.280.2566.
Upon receipt of the Application to Graduate, the Office of Academic Records will complete a Degree Audit. If any discrepancies are noted, you will be contacted via your Ave Maria email.

FERPA Release Form: Under the terms of the Family Educational Rights and Privacy Act, students at Ave Maria University are able to restrict certain elements of their academic records. Please complete this form if you have not already done so and submit it to the Registrar as soon as possible.
In-School Deferment Request Form: This form is to be filled out completely, including any Lender information, and submitted to the Registrar for certification.
Direct Lending Deferment Request Form: This form is to be filled out completely and submitted to the Registrar for certification for those students participating in Direct Lending.
