1- Call the clinic at the number & extension listed above and specify that you are an Ave Maria student who would like to schedule an appointment.
2- Make sure they will accept your insurance. If they do not accept your insurance, you will need to pay up-front and then submit a claims form to your insurance company to be reimbursed. Ave Maria student insurance is accepted. please see Ave Maria Student Health Insurance for more information
3- Make your appointment for Tuesday, between 1:00-4:00 p.m. (if you're using our shuttle to the clinic)
4- Inform the Office of Student Life (ext. 2540 or bridget.mullen@avemaria.edu) that you have scheduled an appointment, so that we can let the clinic know how many students will be coming in.
*This is IMPERATIVE: if the clinic has not heard anything from us by Tuesday morning, they will assume we have no students coming in and will book our time slot for other patients.*
5- Go to the Office of Student Life to pick up and fill out paperwork the clinic requires its patients to fill out.
6- Fax that paperwork to the clinic at 239-658-3074.
7- Students going to the clinic should meet at Student Life; the school van will leave PROMPTLY at 1:00 p.m.

Students who need to be treated at the Immokalee clinic on a regular basis:

A- Must make an appointment with a doctor (steps 1-4) to establish care at the clinic.
B- Must have their primary care-giver fax their medical records to the Immokalee clinic at the fax number listed above, so that the clinic can establish that student there as a regular patient.
C- If the student wants to take advantage of our clinic runs he/she should schedule their regular appointments within the allotted times we offer (Tuesdays, 1:00-4:00 p.m.)
D- If the student has his/her own means of transportation, he/she may schedule the appointments at another time during the week.
